Transition Projects exists to help those experiencing homelessness transition successfully into permanent housing.
With 54 years of experience providing shelter, housing, and supportive programs and services for very low-income people we are recognized for our work with veterans, women, people with disabilities, and other highly vulnerable groups. Our team of over 330 assists nearly 10,000 people each year through programs designed to help people leave the streets, find permanent housing, and retain their housing.
Your gift helps sustain our critical work and day-to-day operations. Donations enable us to add to a strong and sustainable infrastructure and provide the wraparound services that will have the greatest impact throughout our housing programs, our eight shelters, our Resource Center and on the streets through our Navigation Team.
One program specifically impacted by your Give!Guide donation is our health and wellness services provided by Wellness Access Specialists. These team members assist vulnerable populations in their connection to needed medical, mental health, and substance use disorders services for people who have experienced chronic homelessness (one year or more) and are now in permanent supportive housing. Wellness Access Specialists provide engagement, assessment, and one-on-one support to connect participants (our clients) to community-based services. The primary goal of Wellness Access Specialists is to support the long-term health and wellness of participants by ensuring they can successfully connect to and receive the needed healthcare and wellness services which will then create more stability in their housing.
